Covishield Vaccination 2nd Dose Willingness form needs to be filled by students willing to receive their Second (2nd) Dose of Covishield, to be administered at the Vaccination Camp to be held at Siliguri College on the 20th and 21st of December 2021 from 11 am onwards in Room number 4. All interested students are requested to fill up the Google form immediately.

Students are requested to bring their Aadhaar Card, their 1st Vaccination certificate and if possible, the phone number with which they are registered in Cowin.
